Individuals applying for support services must beet the following eligibility requirements:

• Have a permanent physical disability
• Be between 18-65 years of age
• Be cabable, or potentially capable, of directing their own care
• Be able to pay the ordinary costs encountered with living independently
• Be prepared to sign a service contract
• Ensure that any other services required can be provided through other community resources
